# How to collect contact lens prescription?

1. **Create new contact lens form and click edit button**

2. **Publish the form**

3. **Create a new product template**

4. **Add "Contact form" block**&#x20;

5. **Fill in the key and Save**

6. **Modify the product template and Save**

7. **Preview the product**
